Eligibility Requirements for Food Assistance

To receive food assistance, your household must meet the income guidelines in the chart below.

A copy of the Ohio JFC Federal and State Funded Food Programs Eligibility To Take Food Home form can be downloaded by clicking HERE.

 

Permission to Pick up Food

If you would like to grant permission for someone else to pick up your food, please download the form by clicking HERE  and submit the form with a copy of your identification. Bring the form along to your first visit or email us at issexecutivedirector@gmail.com
